The survey evaluated resident care, staffing, cleanliness, food service, and residents’ rights, with only 8 percent of Massachusetts nursing homes achieving zero deficiencies in 2025. The 82-bed skilled nursing facility also holds a five-star CMS quality rating and offers specialized pulmonary, orthopedic, and heart failure rehabilitation programs.
